Families of Infant/Toddlers and Preschool children must complete the following and return it to the Preschool Office prior to your child’s first day.

• Preschool Physician Waiver signed by a physician
• Up to Date Shot Record
  
• Notice of Parental Responsibility
  
• Personal Information Card (2)
  
• Child Care Payment Authorization Form
• Voided check for ACH withdrawal
  

Families of School Age children (kindergarten through completion of 5th grade) who attend the following schools Oakville Elementary, Blades Elementary and Wohlwend Elementary must complete the following and return it to the Preschool Office prior to your child’s first day.
